Approval For Internship Form (IDS 4940)
This form must be completed and submitted to the IDS secretary (Jennifer Grogan) in 2014 Turlington to receive a section number for the course. A five-page report of what you did for the internship is to be typed and turned in along with a daily journal, by (date) so that grades can be submitted and posted by the UF Fall semester deadline.
This course may be taken for 1-3 credits (50 hours equal 1 credit). Grading is on a S/U basis. Please note that a course taken S/U will NOT apply to MAJOR requirements, but may apply to your elective requirements.
